We specialise in palliative care, rehabilitation, aged and dementia care, mental health services for older people and other related health and aged care services.  HammondCare seeks to embed evidence-based best practices in its services and we provide these services through home care, sub-acute hospitals and residential aged care.

The Registered Nurse works within the Mission and Mission in Action of HammondCare to ensure safe, effective, quality patient care.


About the role

Located primarily in the Northern Beaches Specialist Community Palliative Care Team, this role provides palliative care in-reach into residential facilities to provide assessment, care-planning and review. Additionally, this role will include after-hours phone line support provision and case management within the broader community palliative care team.


About you

  • Current registration with the Australian Health Practitioner Regulation Agency
  • 4 years post-graduate experience. Has or working towards a post-graduate palliative care or related qualification.
  • Able to work autonomously within the wider multidisciplinary team.
  • Effective communication and interpersonal skills
  • Organisation, networking and time management skills
  • Able to work with sensitivity, empathy, and flexibility.
  • A current driver’s license and safe driving record. 
  • Willingness to travel between HammondCare’s sites and services.
