About HammondCare

Motivated by mission - not profit or accolades - we strongly believe in the intrinsic value of every person that we care for. We aim to set the global standard of relationship-based care for people with complex needs through clinical research and practice.
At HammondCare we champion life.

About the role

You will join a dynamic Restorative Care Team to deliver high-quality Speech Pathology services to HammondCare clients in the Northern Sydney Community. This is an exciting fixed term part-time role until October 2025 which can be either 1 or 2 days per week with a broad case load.   Whilst you’ll have the autonomy to structure your own day to meet key deliverables, you will also be supported by an experienced interdisciplinary (IDT) team.

Together you will deliver client-centred, evidence-based care in an environment that promotes continuous quality improvement consistent with the values and ethics of HammondCare and Speech Pathology Australia (SPA).

What will your key responsibilities look like?

  • Provision of a high-quality Speech Pathology service to HammondCare STRC (Short-Term Restorative Care) and HCP (Home Care Package) clients including individual 1:1 home visits and group activities.
  • Assess and provide intervention to clients in their homes, and carry out clinic-based or in-home rehabilitation programs to improve functional independence and quality of life.
  • Collaborate closely with our skilled IDT to assess clients’ functional care needs, set care goals, and provide treatment and interventions to deliver the best possible client outcomes.
  • Participate as a member of the HammondCare team in the application of the Mission and values to the pursuit of its objectives in a person-centred, continuous quality improvement work environment.
  • Participate in quality improvement projects and continuing professional development.
  • Provide clinical supervision for students, if appropriate.

About you

  • Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action.
  • A tertiary qualification in Speech Pathology.
  • Current status as an Accredited Practising Speech Pathologist (APD) or provisional APD status or eligibility for APD status.
  • Practical experience or sound knowledge in a Rehabilitation or Aged Care setting, across inpatient/outpatient or community settings.
  • A current driver’s licence and safe driving record, access to your own vehicle and a willingness to travel between HammondCare’s sites and services.
  • Effective communication and interpersonal skills (written and verbal)
  • Demonstrated organisation and time management skills.
  • Demonstrated ability to work independently and as part of an interdisciplinary team.
