About the role
This Level 1, 2 or 3 Social Worker Full-Time opportunity until June 2025 is based at Braeside Hospital, Prairiewood and works within the Mission and Mission in Action of HammondCare to ensure a high standard of Social Work service is provided to patients of the Hospital, along with appropriate support and advice to their carers. The Social Worker will participate as a member of the multi-disciplinary team to ensure a patient-centred, evidence-based, continuous quality improvement work environment that is consistent with the values and ethics of HammondCare and the Australian Association of Social Workers (AASW).
The essence of this role
- Provision of effective and efficient social work service to patients, including appropriate support and advice to their families and carers
- Participate as a member of the multi-disciplinary team in assessing patients’ level of functional and care needs, setting care goals and providing appropriate treatment.
- Participate as a member of the HammondCare team in the application of the organisation’s mission and values to the pursuit of its objectives in a client-centred, continuous quality improvement work environment
About you
- Commitment to align and engage with HammondCare’s Mission, Motivation and Mission in Action
- A tertiary qualification in Social Work, providing eligibility and membership to the Australian Association of Social Workers (AASW). Qualifications obtained overseas must provide AASW Overseas Assessment number.
- Experience across Rehabilitation, Older Persons Mental Health and Palliative Care
- Knowledge and or experience and skills in planning, implementation and evaluation of a range of psychosocial interventions. Demonstrated understanding of discharge planning for patients with a wide range of health conditions in hospitals and community health settings is highly desirable
- Effective communication and interpersonal skills, using a variety of media – oral, computer and written.
- Self-motivated and skilled in effective time management, prioritizing competing work commitments and managing and responding to demanding situations
- Ability to interact constructively and collaboratively with internal multidisciplinary team and external service providers.
- Commitment to ongoing professional development, learning opportunities and supervision
- An unencumbered Driver’s License
