HammondCare Horsley is currently looking for a Permanent Full-time Manager to support our Specialist Dementia Care Program Cottages to join our team.
With our unique model of care which places the residents needs as your number one priority, you will help create a home away from home for our residents. This role requires you to provide tailored care including personal support within the residential home.
About the role:
As the Manager for Specialist Dementia Care Program and Step-down cottages, you will be part of a multi-disciplinary team that supports people living with dementia experiencing very severe behavioural and psychological symptoms of dementia. As a core member of the Horsley team, you will work with a wide variety of people including the Residential Care Operations team, Geriatrician, Allied Health, SDAs (RNs), SDCs (Carer’s), residents and the resident representatives to ensure resident outcomes are met.
The SDCP Manager role is pivotal to HammondCare Mission and Philosophy of Care. Horsley SDCP is part of six other SDCPs operated by HammondCare.
To be successful in this role, you will have:
- Tertiary qualifications in specialised dementia and/or mental health service nursing for older people
- Significant experience operating as a specialist nurse preferably within Aged Care
- Demonstrated ability to support complex behaviours and psychological symptoms of dementia, motivate and develop a team and effectively work with other staff
- Strong leadership and influencing skills
- Strong negotiation and conflict resolution skills
- Ability to work independently and as part of a team
To be successful in this role, you will:
- Facilitate clinical care delivery by the SDAs (Registered Nurses) and SDCs (Carers)
- Able to provide hands-on modelling, coaching, and mentoring in clinical and complex behaviour management areas and processes
- Support resident case management and clinical decision making
- Maintain clinical oversight and professional competence as a Registered Nurse with experience in complex dementia-specific care
- Support and investigate clinical/resident incidents as required
- Communicate effectively with all stakeholders, both within HammondCare and external partners to see the best outcomes for the residents
